Clip Chart Individual Behavior Note

Already I am aware of students who need individual behavior plans and I want them to tie into our class clip chart.  As well as be responsible for explaining how they ended up where they did.  That way when they get home, it is a bit harder to "forget" what their choices were during the day.  Here is what I came up with.  It will probably need some adjusting, but it is a start.  This particular chickadee is working on staying at ready to learn or above 3 out of 5 days this week.  If they do it, they get to choose from the reward binder.  Click here or on the picture for your own copy.
Happy Wednesday,

  1. Hi Laurie,

I use a behavior chart similar to this. I am planning on sending home an end of the week behavior report to parents. This is perfect. Thanks.

  2. Nice! I send a color coded sticker home each day, so that parents know where their child ended up! Twelve days and only one home contact! This is the first year I've used a clip-chart...I love it! If the whole class can stay on ready to learn or good as gold we add a letter to spell popcorn. When we have spelled the whole word we will watch an educational video and eat popcorn!
